안와골절 재건후 발생한 반흔성 하안검 외반증에 대한 증례보고

Abstract
결론
하안검의 외반증은 여러 가지 원인에 의해 일어날 수 있다. 안와 하연과 안와저 부위를
수술하는데 있어서 사용되는 접근방법이 다양하고 이에 따른 술후 외반증의 발생 위험성도
접근 방법에 따라 다양하다. 또한 술후 발생한 외반증에 대한 심미적이고 기능적인 수복이
이루어지도록 해야하겠지만 무엇보다도 술전에 환자의 상태에 대해 충분히 진단하고 수술중
에 외반증 발생의 예방을 위한 주의를 기울여서 시술하는 것이 중요하다고 하겠다.
본교실에서는 안와 골절 수술후 발생한 안검 외반증을 검판대수술로 재건하여 심미적으로
나 기능적으로 양호한 결과를 얻었기에 문헌고찰과 함께 보고하는 바이다.

Ectropion is a drooping or eversion of the lower eyelid as a result of lower lid laxity,
canthal tendon laxity, or cicatrix formation from scarring. Ectropion can also be
congenital or develop with aging. This complication can arise from primary trauma to
the lower lid apparatus or as a result of unfavorable healing and scar contracture.
To prevent development of ectropion, a careful attention in case of operation for orbital
frature repair or blepharoplasty is necessary.
Especially, excessive dissection or retraction of skin and orbicularis muscle should be
avoided, and formation of edema and hematoma should be prevented by complete
hemostasis. It might be helpful to massage on the eyelid frequently.
In spite of endeavors mentioned above, lareral canthoplasty or skin graft must be
performed when postoperative ectropion is occurred.
With literature review the authors present a case report that good clinical result is
achieved by lateral tarsal strip procedure on cicatricial ectropion caused by a
complication of reconstruction of blow-out fracture.
